Information about weight management services and the availability of weight-related drug treatments in Suffolk and North East Essex:

We are aware of the recent news reports, however we are not able to prescribe this in General Practice locally, this varies by area – in this area there is a new NHS weight management service expected to launch later this summer. Full details aren’t available yet and we are not yet able to refer for this, but it’s likely to include weight management programmes, medications, or surgery depending on individual assessment once you have been referred on to them. Most patients will need a BMI over 35 and other conditions to be referred and the team will assess and decide on which will be suitable for each individual. CONFIDENTIALITY STATEMENT:

Set out below is the Practice Confidentiality Statement. Please read very carefully and clarify anything you do not understand:

  • I understand that in the course of my visit to the Practice I may have access to personal information, medical and otherwise, relating to patients, staff and other people who visit or telephone the Practice.
  • All information that I come into contact with is of a confidential nature and must be treated accordingly. I am not allowed to discuss it or disclose this information to any other person without the permission of a GP or the Practice Manager.
  • I understand that all information passed on to me, including that which may be about a member of the Practice staff, may only be passed on to those persons who are entitled to receive this information in pursuance of their duties.
  • I understand that information about patients (including names, addresses, telephone numbers, whether they are waiting at the surgery or have left, past medical history) is not to be given to anyone.
  • If I have been given authority to pass on information to someone then I will pass on that information only and I will make sure that the person to whom I am passing the information is authorised to receive such information.
HEALTH & SAFETY STATEMENT:
  1. Action upon Discovering a Fire: Sound the Alarm by operating the break glass switch. These are located; Rear Entrance, Reception wall by the receptionist desk, End of the consulting corridor, Nurses’ Corridor.
  2. Evacuate the Building. Fire Exits are located; The Main Entrance, Rear Entrance, End of the consulting corridor, Nurses’ Corridor.
  3. Assembly Point: At the rear of the practice in the car park.

Do not re-enter the building until the Fire Brigade/Fire Warden has announced that it is safe to do so.

Fire Extinguishers can be found around the building but should only be used if safe to do so.
